Atlanta Dog Trainer LLC 12900 Freemanville Road, Alpharetta, GA 30004

This Waiver and Release Agreement (the "Agreement") must be signed and dated prior to participation in Obedience & Agility Classes with Atlanta Dog Trainer LLC, whether as a participant, visiting guest, apprentice, subcontractor, trainer, or any other capacity on the property. The undersigned acknowledges that this Agreement covers and binds both the signatory and any accompanying persons who may attend classes or be present on the property.

1. ACKNOWLEDGMENT OF RISKS

The undersigned acknowledges and understands that participation in Obedience & Agility Classes at 12900 Freemanville Road, Alpharetta, GA 30004 involves inherent risks, including but not limited to:

• Interaction with other animals
• Physical activities requiring running, jumping, and climbing
• Use of training equipment and facilities
• Potential for injuries including scratches, bites, falls, and trips
• Possible transmission of diseases between animals
• Environmental hazards on rural property including but not limited to:
  • Equipment and tools
  • Wildlife (including snakes, raccoons, squirrels, coyotes)
  • Natural terrain and obstacles
  • Various training and agility equipment

2. HEALTH REQUIREMENTS

The undersigned certifies that their dog is current on all required vaccinations, including:

• Rabies
• DHPP
• Bordetella

3. RELEASE OF LIABILITY AND COVENANT NOT TO SUE

In consideration for participating in Obedience & Agility Classes, the undersigned hereby:

a) Releases, discharges, and covenants not to sue Atlanta Dog Trainer LLC, its staff, agents, administrators, heirs, contractors, maintenance staff, trainers, officers, directors, and other participants (collectively, "Released Parties") from any and all claims, demands, actions, judgments, and executions which the undersigned ever had, now has, or may have, or which the undersigned's heirs, executors, administrators, or assigns may have against Released Parties for any injuries or damages, both foreseen and unforeseen, known and unknown, sustained while participating in Obedience & Agility Classes or while present at 12900 Freemanville Road, Alpharetta, GA 30004 for any purpose.

b) Assumes full responsibility for any financial obligations resulting from any injuries or damages to themselves, their pet, or property while participating in Obedience & Agility Classes or while present at 12900 Freemanville Road, Alpharetta, GA 30004.

c) Agrees to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs they may incur due to the participation of the undersigned or their pet in Obedience & Agility Classes or presence at the facility.

d) Acknowledges and agrees that this release extends to and covers all accompanying persons who may attend or participate in activities at the facility in connection with the undersigned, including but not limited to family members, friends, caregivers, assistants, and observers. The undersigned assumes full responsibility for communicating these terms to all such persons and accepts liability for their actions.

4. MEDIA RELEASE

The undersigned grants Atlanta Dog Trainer LLC permission to use any photographs or videos taken during Obedience & Agility Classes for promotional purposes.

5. ACKNOWLEDGMENT

• I have read and fully understand this waiver and release of liability.
• I understand that by signing this agreement, I surrender substantial rights.
• I sign this agreement voluntarily and without inducement.
• I understand that group class tuition is nonrefundable.
• I understand this agreement applies to all activities at 12900 Freemanville Road, Alpharetta, GA 30004, whether as a participant, spectator, guest, or visitor. I understand and agree that this waiver covers and legally binds not only myself but also any accompanying persons who may attend classes or visit the facility in connection with me, whether or not they have a familial relationship with me, and I accept responsibility for informing them of these terms.
